New Patients

If you're calling for the first time, please tell us that you're a new patient.

Cancellation Policy

Please call us at least 24 hours in advance if you need to change your appointment so we can schedule another patient at that time. $50.00 charge for cancellations in less than 24 hrs. Same fee for a no-show.

What to Bring with You

Please bring your insurance card and a list of any supplements and or medications you are taking. You also may find it helpful to bring any medical records in your possession or a list of questions you want to ask your doctor or nurse. If you are a new patient please allow fifteen minutes before your appointment to fill out your paperwork.

Health Plans / Insurance

Cottage Clinic Providers participate with a number of insurance companies. Insurance companies typically have more than one product or plan.

As a patient it is your responsibility to contact your insurance company for the most current provider and benefit information on your policy prior to your appointment.
